Registration Information
| Registration: |
Registration will occur via the TRAIN learning management system. For participants within Ohio, log on to http://oh.train.org and search for Course ID 1011218. For participants outside of Ohio log on to http://www.train.org and search for Course ID 1011218.
If you do not have a TRAIN user account (user accounts are free):
1. Click the Create Account button on the left menu.
2. Fill in the required fields as instructed, and then click Submit.
3. Record your username and password for future reference.

$675 - courses without evening laboratory sessions
$775 - courses with evening laboratory

| Tuition Support: |
The College of Public Health is actively pursuing external support for this program. Tuition support in the form of full or partial scholarships may become available for qualified applicants. Please check back regularly for tuition support information.

| Registration Fee: |
There is an $80.00 non-refundable registration fee on all applications.
|
| Payment: |
Payment instructions will be included with your OhioTRAIN registration confirmation. Online payment by credit card is available.
|
| Deadline / Late Fees: |
The deadline for registration is Monday, June 23, 2008. A late fee of $150.00 will be charged to all registrations postmarked after June 23, 2008. No registrations will be accepted after Monday, June 30, 2008.
Note: If you are currently enrolled as a student at The Ohio State University and would like to receive academic credit for your course(s), please call Meredith Cameron at 614-292-2291 or Shirley Funt at 614-292-1637.

| Cancellations/Refunds: |
All cancellations will be charged an $80.00 cancellation fee. Refund requests will not be honored if received Wednesday, July 2, 2008 or later.
